EPISODE EXPANDS TO FIT IDENTICAL TWINS TEAMS — The competitors in this 90-minute grueling, four-stunt episode comprise four sets of identical twins who must work in sync as teams if they want to finish first and win up to $50,000. Their first stunt would make anyone "see double": secured beneath a helicopter over a frigid lake, one twin has to jettison the other into the water and then must jump in too. After a harrowing swim, they both have to dive off of a moving speedboat. FORMER CHAMPS RETURN – A half-dozen previous winners return in this episode and must ride a bicycle across a 6-inch-wide beam bridging two speeding semi-trucks in addition to having to turning mouthfuls of tomato horn worms into juice to be quickly downed. The returning winners are: Ari Sturm, the winner of the first episode ever, who used his winnings to travel around the world; native Barbadian Olivia Jones; Summer Papania, who milked a goat with her mouth in the episode she won; Jeanette Jenkins, Queen Latifah’s personal trainer and the first female winner ever; Noel Holmes, the first openly gay contestant and Randy Huston, the "Cowboy up!" winner. Friends Let Friends Drive Blindfolded – The results are a smash when this episode features four pairs of best friends teaming up in a "Fear Factor" first: a passenger directs a blindfolded friend to drive their car up the back ramp of a moving automobile carrier and park inside. WHEEL FUN — This episode features two outrageous motor vehicle stunts: first, contestants have to snag flags attached to two poles jutting out from the front of a moving dump truck; and second, they must grab flags placed on a car turning as if on a "rotisserie" and suspended 20 feet over a lake. BROTHERS AND SISTERS, IS FEAR A FACTOR? – This episode features four pairs of siblings (comprising two sets of brother and sister; two brothers and two sisters) as the contestants. The duos’ challenges include one of the show’s signature "gross out" stunts: the pairs each have their heads multi-locked inside a small glass box filled with hundreds of large and small spiders, and must perform many mouth-to-mouth key transfers to unlock themselves from their tangled traps. Included in this action-packed episode are two of the series’ most breathtaking stunts. In the first stunt, contestants attempt to unfasten the front of the glass diver’s helmet secured over their heads and filled with angry Moray eels. Another high-flying challenge is an aerial flag-grab over Southern California below a sea-plane borrowed from Grammy-winner Kenny G.
Next season, NBC’s "Fear Factor" is leaving Los Angeles to spread out across the U.S. and invade cities and towns in order to challenge fans to face their fears in an actual "Fear Factor" stunt on the fans’ home turf – their house. "Capital One’s ‘Fear Factor’ Home Invasion," part of a number of exciting innovations the series will add for the next season , includes a battle plan to visit 22 cities nationwide.

This one-time re-match reunites the five couples from season four’s successful couples series. One of their spectacular challenges requires a partner to walk a narrow beam 10 stories high, retrieve a key, and send it to their partner waiting below on a Jet Ski which has to be launched off of a three-story parking garage.
